• Cancellation Policy:  2 days minimum

• Late Policy: 10 minutes maximum

• Full or inactive voicemail

• Name on your ID and insurance card do not match

• Address on your ID and insurance card do not match

• Hanging up the phone, using profanity, being confrontational
